Fees:
- $125 intake paid directly to TDHC at intake which will occur at TDHC clinic (60 Min Session)
- $100 a month
- Includes 3 office visits at SCC or TDHC location (approx one every 4 months)
- Up to 9 included E-visit via phone or video (on months not in person)
- Texting access through TDHC platform
- Member discounts on labs and in office needs.
- 50% non-mental health need visits (ie UTI ect) ($25-20 min, $35-45 min or $50-60min)
- 50% off additional mental health visit needs for $50 per visit
- If a client is on controlled substances they will have to be on a membership plan to receive services.
- $10.00 for superbill outside of intake, $15.00 for more than one superbill with same day request
